Dead Ball on an Appealed Third Strike?

Does an appealed 3rd strike constitute a dead ball?

No, the ball stays live on all strike appeals unless the ball hit the batter and he swung at it and the defense is appealing the swing. This is because anytime a pitched ball hits that batter the ball is dead.
